How To Report A Workers’ Compensation Claim

We can guide you through the process of reporting a workers’ compensation claim, which will include completing an FROI. While you have two years to complete the report, it is wise to complete it as soon as possible.

To report your injury or illness, you must complete the form titled “First Report of an Injury, Occupational Disease, or Death.” Your employer may be able to provide this form, but it can also be completed online. In the FROI, you will fill out a detailed description of the accident, a medical diagnosis if you have received one and other information relevant to your claim. After filing your paperwork, the BWC will issue you an identification card with your claim number and other pertinent information. The ID card can be used in a manner similar to a health insurance card when you are obtaining medical services.

The BWC will inform you of a claim allowance or denial within 28 days of submitting your FROI. If you are denied, you have further options to appeal the decision.
