What information do I need to apply for SSD in Ohio?

Being disabled and unable to work is difficult enough for most Ohioans. Then, to receive benefits from the federal Social Security Administration (SSA), you have to fill out an application with all the correct information needed to ensure you meet the requirements for Social Security Disability Insurance (SSDI). This application can be made a few different ways, including in person, over the phone or online. In any event, there is some basic information you will need if you want to create as complete an application as you can.

First, you will need information about yourself and your family. This will include such things as your own legal name, date of birth and social security number. You will also need the names dates of birth and social security numbers of all your spouses, past and present, as well as the same information for your minor children. Finally, if you want to have your benefits deposited electronically, you will need your bank account information, such as account and routing numbers.

Second, you will need information about your medical condition. This would include the contact information of anyone who knows about the condition and can be of help if contacted by the SSA. Importantly, you will need information about your illness, including patient ID numbers and dates of treatment, along with names addresses and phone numbers of the doctors you have seen. You should also have a list of your medications and the prescribers thereof, and the same for any tests you have undergone.

Third, SSA will want information about your employment. This includes names and addresses of employers for the last two years and the amount of income you earned over that time period. A copy of your social security statement, and a list of up to five employers you've had over the 15 years prior to commencement of your inability to work may also be required. You will also need the beginning and ending dates of military service prior to 1968 and information about any workers compensation or other benefits you have received.
